A pesar de que, durante una época, se limitó la aplicación del entrenamiento asistido por robots al tratamiento de personas con lesiones neurológicas (en su mayor parte, personas que sufrieron ictus), en los últimos años se ha comprobado su funcionamiento en otros trastornos neurológicos, tales como la esclerosis múltiple, la parálisis cerebral, la lesión medular o el traumatismo craneoencefálico.

Por tanto, ha resultado innegable, tras los resultados obtenidos, la eficacia de la terapia en la mejora de la fuerza muscular, la función motora y de la independencia en tareas cotidianas.

La primera vía se centra en el tipo de movimiento que se puede realizar. Comprende, así,  el diseño de sistemas robóticos que permitan movimientos más naturales. El objetivo último de estas prácticas es mejorar la independencia del paciente, para lo que se quiere la autonomía para realizar movimientos funcionales, lo que se consigue mediante un tipo de entrenamiento motor que da resultados con la repetición del ejercicio.

Este tipo de ejercicios implican un mayor recorrido de movimientos y, por tanto, sistemas robóticos más sofisticados, con múltiples grados de libertad, con el objetivo de que el paciente pueda realizar el ejercicio aplicando un gran número de grados de libertad en las articulaciones del brazo y de la mano.
